PSC- elevated enzymes- why are they elevated and how to make them go down

Hello,

I was diagnosed with PSC in conjunction with UC at 16yo (currently 23yo). I am currently having a moderate flare, though I am on UCERIS to get it under control. My GI doctor ordered routine blood work and my liver enzymes came back very high. What is the cause of the liver enzymes being high?

I have read that being on 6mp can make liver enzymes high, but when this blood work was done, I had not been taking 6mp for 5 months (so it was most likely out of my system). I am now taking it. Could being in a moderate flare create a spike in liver enzymes? Or, could this be a sign of worsening PSC?

I have other symptoms such as stomach pain, fatigue, dizziness (this one specifically is new from the past 6-8 months or so), and at times left-sided pains stomach/abdomen pains (not common and not very severe). I do not know the exact cause of these symptoms...could any be related to the PSC? Just want to get as knowledgeable as possible and know if there is anything I should keep in mind to tell the liver doctor during my appointment in January.

Ask about probiotics and fecal transplant.

Currently there is a drug (FXR antagonist) in clinical trial being tested for PSC and other live conditions.

The activation of FXR inhibits bile production, increases glucose, fat, and bile absorption. FXR activation also protects liver from fibrosis and other harmful effects.

In Canada, there is currently ongoing investigation into the role of microbiota and autoimmune liver conditions.

Look at this recent study.

"Patterson said that, according to this study and earlier research, tempol and antibiotics appear to reduce some types of Lactobacillus — an intestinal bacteria that converts lactose and other sugars into lactic acid. When Lactobacillus levels decrease, a bile acid — tauro-beta-muricholic acid — increases. The increase of this bile acid then directly inhibits FXR — farnesoid X receptor, which regulates the metabolism of bile acids, fats and glucose in the body, according to the researchers."

Basically lactobacillus activates the FXR.

There is also this study showing that an imbalanced gut bacteria can lead to liver damage.

I would HIGHLY suggest you get a fecal transplant immediately. Increasing lactobacillus will only be helpful.

FMT could be a viable treatment for liver conditions such as PSC.
