I had a colonoscopy in 9/13 and was diagnosed with moderately severe colitis in the rectum. Had another colonoscopy today and was told I have moderate colitis of the rectum and sigmoid colon. Does that mean it has spread? Is it likely to continue spreading? I've been taking Apriso but doctor now wants me to take Humira since Apriso doesn't seem to be helping. Thank you!

If your disease was limited to the rectum in 2013 and now it is in the sigmoid colon, then yes, it seems to be spreading.

I would suggest using rectal mesalamine retention enemas in addition to the Apriso. This medication is meant to treat inflammation in the sigmoid colon and rectum.

Yes, uc spreads when your medications are not sufficient. Often immunosuppressive medications (Imuran, 6mp, aza) are tried before humira.

Personally I would switch the Apriso to one of the many other 5asa's and start rectal meds before taking immunosupressives or biologics. And I would get a new doctor. This doctor will have you doing surgery before you know it. Have you taken any probiotics, supplements, or done any diet changes?

I was using a mesalamine suppository for a long time. After a pretty bad and lengthy flare and discovering that the colitis had spread through my entire colon, my doctor changed me from the suppository to the mesalamine enema and Apriso. I had another flare about a year after that and found that only part of my colon was flared up. So, I did another round of prednione and he then switched me to Delzicol (another 5ASA) and continued the enema. I have been in remission for a while now. I actually prefer the enema to the suppository. I don't have a problem (most of the time) keeping the enema in over night. I think that there are other options to try before going to Humira. See if your doctor will consider some other things first.

sburg...the doc should have put you on an oral 5ASA in conjunction with enemas AND suppositories initially.

I'd push for rectal meds asap to try before you're heading into the biologics or pred. You can always add steroid rectals before the systemic pred.
A few options at least.

Do you have the biopsies pathology report from when you were diagnosed? If not, get it....because that might tell a bit whether there was some cellular changes above the visual inflammation.
As well, how much of your rectum was initially involved? The reason is that if the part above the severe was moderate...it was already showing progression.
