New drug XELJANZ for Chrons and UC

It's a tablet taken twice a day, shown to stop bleeding in two weeks and in 4 weeks less stool trips this doctor said. Right now it's approved for RA . I don't qualify really for anti map because of my fistula. Although he said I could still maybe do it. I haven't seen much improvement from entyvio. What is convient for me is the tablet and not having to do infusions. Xeljanz has completed one study and is on phase two of another. Same side effects apply as if we're humira or any other biologics, cus cause meningitis , fungal infections, cancer etc....I meet with my GI and I'm going over my options on Friday. I still have bloody diarreah every day, no formed stools and no energy really. I can't jiggle my gut, like walking, it stimulates poop in a urgent bloody mucus way. I know exercise can but this is that way. Which is safer these types of medication or taking a bunch of antibiocs ? So confusing.

I've been taking a bunch of antibiotics for the past 12 months for pouchitis, sometimes they wear me out and cause other problems like Thrush and SIBO. What does help me is rotating antibiotics, it really keeps the side-effects down.

I think they've been prescribing Xeljanz for a while now off-label for the tougher cases.

My GI recently said that he began using Stelara for some of his patients.

Xeljanz is an anti-tnf biologic treatment just like humira, remicade or simponi. Not yet approved for uc, last I knew. More convenient as no needles involved, it's an oral pill.

I heard Xeljanz had some issues with stomach bleeding, if memory serves me right. Probably low odds, but something to be aware of.

My GI had some people in the study for xeljanz and she said none were having great response. I believe they were all people who had failed other anti-TNFs so that would explain why. I would have tried the study but prior use of entyvio was not allowed. I think I would have had to stop entyvio for 6 months or something to get into the study and i didn't want to suffer for another 6 months. Besides the point, I didn't have luck with antiTNF so why would this on be any different.

Sorry to hear you're not doing better on entyvio. You seem to have similar UC to mine, non-responsive to anything.

There is a stelara trial but i think it's for Crohn's. I doubt it would have helped me. Not to be negative but i think I have something different from the majority.

It is a JAK inhibitor.....not an anti-TNF. Its a completely new class for IBD

all I know is that my GI said none of the patients she got into the trial had luck. Granted they were all people who didn't respond to any of the other meds (anti-tnf, entyvio)...

It's used for treating rheumatoid arthritis just as Remicade is used for treating RA. So it may be hopeful for some UC sufferers but probably a small percentage.
