As Remicade is most often used in the US, you continue getting infusions indefinitely. Stopping the medication often results in return of a flare, and often Remi doesn't work a second time. Some people do choose to go off and do well, however this isn't the usual process.
An infusion means the drug is given intravenously. An IV is started, a bag of fluid containing the medication is hung and infuses into the vein over about two hours. You relax in a bed or recliner chair during the infusion. Then the IV is removed, an Band-Aid is applied and you go home.
I get blood work quarterly, which is the same as with any other UC medication I've been on (except for Azathioprine, which required more frequent tests.) Serious side effects from Remicade are actually pretty uncommon.
