When does the horrible cramping pain occur? Is it during bms? Randomly? This makes me think of muscle spasms within the large intestine (very painful) and those can be treated with prescription anti-spasmatics that your gastroenterologist can prescribe. In the meantime, I'd highly recommend a hot water bottle (or heat retaining pad) on your abdomen when pains occur, it greatly helps.
I'd give the mesalamine enemas couple weeks to show improvements and ask for corticosteroids if things get worse. Severe flares usually warrant prednisone, uceris, or a steroid enema.
We've all been there. My worst flare was 20+ bms a day, sharp knife stabbing pains, the works. It sucks that any one of us need to suffer like that due to uc symptoms, worse yet that we all do during bad flares.
