Day 2 after diagnosis...are path reports correct for disease activity?

Can someone explain to me how to translate "symptoms v path report v disease progression"

So I have inflamation in all parts of my colon. Therefore, I have panacolitis. This is supposed to be the most severe form of UC.

But the path report suggests "mild" activity in some parts of the colon...like ascending colon, and some areas of "moderate" activity such as cecum or transverse colon.

And finally, I have had symptoms on an of for 2-3 weeks, with several days of formed stools and feeling fine. Based on the severity physical criteria, I have "mild" phenotype currently with less than 4 stools, and only occasional drop of blood.

What does this mean? Is the disease activity on the path report more suggestive or the area of the disease.

these are related by not directly connected topics.

pancolitis often does mean UC, but not unless path report says UC - otherwise just actue inflammation all over.

pan is about where.

UC is about chronic.

mild/moderate/severe is about intensity of inflammation

progression depends on UC or colitis and on treatments

symptoms generally go up in association with rise in inflammation, but some people have symptoms not connected as strong to inflammation.

Oh my path report says "UC", doesn't really say "pancolitis". The doctor called it that.
But most of the path report says "mild" activity in each portion of the colon, yet it is in the whole colon. So I guess I am asking what si the difference between lets say severe disease activity in just one or two parts of the colon v. mild disease activity in the whole colon.

Ulcerative colitis is measured in both extent and severity. You have the furthest possible extent of UC with pancolitis. Severity of UC inflammation is measured as mild, moderate, or severe. You describe mild or moderate severity. Severe inflamation is 10 plus bms a day.

Severity of inflammation is actually the biggest concern with UC, much more than extent. Having mild pancolitis is much, much better than having severe proctitis as far as quality of life and symptoms go. We've had members have colectomy surgery due to a quarter-sized patch of severe inflammation that will not respond to any medication. We have members with mild pancolitis who take very mild meds and have great quality of life and a well mannered case of UC that is little trouble.

Now, if you have severe pancolitis then it's extremely not fun and you might be hospitalized.

Here is the exact verbage on my report.
Granulomatous reaction is seen in close proximity to crypt destruction, favoring mucin granuloma (I was told mucin is nonCrohn finding). Although Crohns disease can not be definitely excluded, the overall picture suggests Ulcerative colitis".
Pretty clear UC. To be honest, Im not sure why they put the Crohn's comment as there is not a single Crohn's marker in the findings.

But definitely UC.

This makes more sense. Yes, it seems like my symptoms and clinical picture are mild to moderate pancolitis for now.

Doesnt say UC ad diagnosis.....it says suggests. Indeterminate colitis is possibly where you could sit for a while since CD cannot yet be crossed off the list.

Where exactly is the cryptic burst?


Regardless....you should be on meds that cover all areas of inflammation.

I totally understand your need for understanding diagnosis and pathology. Been there, although not with UC diagnosis...mine is pretty much classic symptoms and not throughout.
