Help- rectum bleed, cortifoam or mesalamine enema?

Hi All,

I have UC for 3 years and I am currently on a flare since August after a colonoscopy-sort of. My blood test and my stool test show I don't have any inflammation now, but I still have the symptoms like going to the restroom 3,4 times a day, a lot of gas. My stool sometimes is firm but sometime is not. My stool usually has blood at night. I am guessing my rectum is still inflamed. I alternate cortifoam and mesalamine enema every night. But I cannot sleep well when using mesalamine enema, very painful. Which medication is stronger? Cortifoam or mesalamine? I saw the instruction on cortifoam is only used for 2-3 weeks, but I have been on it for almost 3 months, has anyone know if it is ok to continue to use it? Does anyone know any better medication to treat rectum inflammation?

Hydrocortisone (enemas or suppositories) help me a lot more than mesalamine. Used them for many months at time. But ask your doc about long-term use.

Steroids are always stronger than mesalamine. Long term use of any steroid is a bad idea. Rectal route steroids are 90 percent topical and 10 percent systematic, so you are getting some body-wide affects. Steroids taken rectally, thin the rectum walls over time.

Ive had luck with two mesalamine enemas a day (rowasa 4.0 grams times 2).

Yes. When my UC was at its worst I was doing them every night. Then every other night etc etc

I had very bad bleeding with my UC was at its worse. I took prednisone which stopped the bleeding and was also taking mesalamine 6 to 8 pills a day. Then I developed a lung problem from the mesalamine so doctor had me stop taking it immediately. Lungs have pretty much returned to normal since then 5 years ago. I never felt the mesalamine helped me at all. Once I stopped the prednisone bleeding would return. So much blood toilet was always red after going. That is when we decided to remove my colon.

Have found that 40% zinc oxide (diaper rash ointment) has helped my rectal burning more than any other medicine I have tried and I have tried a LOT of them.

I went from once a day to every other day, then to every third day, then to zero.
