As others have said, Entyvio Connect reimburses you for the cost of medication only (not the cost of setting the IV, saline, or attending nurses time). Look at your EOB and see what the costs were for, and you're eligible if it meets the aforementioned criteria. Fortunately the medication is the largest cost by far. The other expenses are usually minor.
If you haven't done any claims before then contact Entyvio Connect and see what they need you to do to submit a claim for reimbursement. All these programs are a bit different, they make the rules, follow them precisely to get reimbursed quickly and with as little hassle is as possible. At minimum you will need your EOB. You can likely mail it in, or hopefully by now submit it electronically lol. They can cut you a check, or give you a refillable reimbursement debit card (if it's like RemiStart). The refillable reimbursement card is handy, if you expect to need reimbursement often. They'll add the balance electronically and that cash is available for use a lot sooner then a snail-mailed check is.
