If you've maxed out the mesalamines at 4,800mgs orally, and 4,000mgs rectally via a rowasa enema without relief then it's time to consider:
1.) Immunomodulators (azathioprine/imuran/6mp) which are generic, oral pills that take 3-months to work. They require blood monitoring the duration you are on them. Gasteroenteroligists are starting to leer away from these, as they have a very small (less than 1% odds) chance of causing a lymphoma (predominately adolescent males affected).
2.) Biologics like Remicade/Humira/Entyvio are injectable, brandnamed medications that work in 3-6 weeks, and do not require blood monitoring. I've been on remicade since 2012, have had zero side effects, and have experienced remission from it.
I'd discuss both with your gasteroenteroligist. The Crohn's and Colitis Foundation of America has a good risk versus benefit watch/read, if you're concerned about
