Restrictions in use of Xeljanz while EMA reviews risk of blood clots in lungs
Press release 17/05/2019
EMA’s safety committee (PRAC) is recommending that doctors must not prescribe the 10 mg twice daily dose of Xeljanz (tofacitinib) in patients who are at high risk of blood clots in the lungs. These include patients who have heart failure, cancer, inherited blood clotting disorders or a history of blood clots, as well as patients who take combined hormonal contraceptives, are receiving hormone replacement therapy or are undergoing major surgery. In addition, doctors should consider other factors that may increase the risk of blood clots in the lungs including age, obesity, smoking or immobilisation.

That's why my dr pulled me off the high dose. If i flare and need to increase again i will have to stop taking contraceptive pill.

Birth control pills plus UC seem risky on its own. We are at increased risk for thrombosis as it is. I started taking the mini-pill because of it.

If I understand correctly, a good part of the thrombosis risk is due to active inflammation (which i don't currently have, as far as I know)**. I would almost certainly get acne from the minipill sad I am considering stopping combined pill, but again, afraid I will get acne, which is the reason i started it in the first place...

** edited to add: because last year I wanted to do an intercontinental flight while in a flare and the hospital told me not to, due to risk of DVT when there is inflammation

It's interesting that so many of us deal with pimples. We must all have crazy hormones. I got an IUD and that doesn't help much with pimples, but I was taking cerazette minipill and that was great for my skin. It might be an option for you.

I have migraines so I was not comfortable with estrogen + UC + migraines + on and off steroids.
