Colitis bleeding v hemorrhoid bleeding

Hi!

I’m starting to think my bleeding is from hemorrhoids instead of colitis. I was in a mild flare with bleeding at the beginning of the year. As it wasn’t going away, I put myself on strict SCD and started taking LDN. Within 3 months, my calprotectin levels went from 185 to below 100 (which is normal). My GI did a sigmoidoscopy as she said it looked good. I do have left sided colitis so she didn’t get all the way up. So everything looks good with labs and testing but I still see blood. I mostly see the blood in my evening BM. I usually have 2 to 3 BMs a day. The blood is bright red but I know that my colitis blood can be bright red too. The blood is usually piled up as the first stool passes through and then it’s on the toilet paper. However, this isn’t every day. It’s random days. Could this be hemorrhoids? As I have no other symptoms of colitis. It’s just so scary seeing blood and I just automatically always think it’s colitis but I m starting to think I’m worrying for nothing ugh! I do have an appt this month with my GI so hopefully she can clear things up soon!

I think it’s very possible it could be hemorrhoids. Sorry you have to wait to see you GI.

I'd just start over the counter hemi stuff for a couple weeks and see if it improves or not. Then you know. Happened to me, got endoscopy as I was worried about blood in stool. Doc said I was sill within a remission lol. Got some hemi cream (nossle goes in Sphincter and squeeze)and it cleared up the bleeding within a few weeks.

Can you use steroid supps? Whatever it is, I am thankful it's mild. I would think if your GI did a sig-scope recently, she would have mentioned if there were hemorrhoids. The low calprotectin is a good indication that things are going well. Maybe you could repeat the calprotectin, or try some supps, and see if the symptoms disappear. Steroid or uceris rectals could be used to treat either condition.

What medication are you taking for treatment and maintenance of your left-sided UC?

Thanks for all the responses.

I am taking ASAs - Mezavant and Pentasa enemas, LDN and strictly on the SCD diet. My GI was pushing biologics on me and I don’t feel I need those. I put myself on SCD and LDN and afternoon few months my calprotectin dropped.

I have never taken steroids, oral or rectally. I don’t think supps will do anything for me because the very bottom looks clear with the scope. It’s usually at the bend that’s hard to get to. I asked my doctor about prednisone but she refused to give it to me. I’m not sure why. She said she didn’t want me taking a lot of steroids if I’m refusing biologics.

I have symptoms very similar to yours. Recently went to my GI for a checkup. I had been in remission for 18 months and then started to get some bleeding and discomfort that I associated with hemorrhoids - that matched the description of your symptoms.

After the checkup the doctor determined I have some mild proctitis which is inflammation only in the lowest part of the rectum. This inflammation can also trigger or aggravate hemorrhoids. Summary - it’s a very mild flare.

I jumped on it like if I was having a full flare. Tightened up my diet, supplements and resumed ASA suppositories which are working very well. I also cycle a lot which can aggravate hemorrhoids so I changed my exercise routine to avoid cycling for 6 weeks and I am very close to having no more blood.

Approximately same here. The only symptom I have lately is bright red blood, usually only on toilet paper. No diarrhea, no pain or cramping. Blood tests also in check. Thanks for the input GoodbyeUC now it makes sense, I was wondering if it was hemmies or a flare, the mild proctitis scenario seems valid. I confirm that mesalazine enemas are quite effective for such mild flares.
