Prednisone dosage

I would say 40 mg until remission, probably 2 weeks for most people, then drop to 30 for a week, and then drop by 5/week until off. That would be 8 weeks total. I've never heard of a 2-3 week course being enough to put someone into remission.

"For moderate to severe UC, the preferred initial prednisone dose is 40 mg/day administered as a single dose. The optimal tapering strategy has not been determined, but experienced clinicians will typically treat the patient with prednisone 40 mg/day for 2 to 4 weeks, then taper by 5 mg/week to a daily dose of 20 mg/day, then slow the taper to 2.5 mg/week until prednisone is discontinued."

I will add, like i said via email, I almost always tapered by 10 mg per week after being at 40 mg for 2-3 weeks. There were a few times I tapered by 5 mg every 5-7 days.

Is it ok to start at 30mg instead of 40?

Make sure you stop your Uceris when you start the pred. Not sure if your rheumatologist told you that.

I tried starting at 30 mg. It just increased the length of time I had to be on pred. I am now of the belief that if I need pred I hit it hard (min 40 mg, last time I had to bump up to 60mg) to start and then taper by no more that 5 mg every 5-7 days for best results.

For me split dosing works best so I have continuous levels of pred in my system. Didn't really mess with my sleep and it helped to minimize side effects, When I took the full dose at 1 time it was wearing off after about 12 hours and my symptoms would start increasing until the next dose. When I changed to split dosing, I steadily started to improve.
