I unfortunately developed a hypersensitivity to all mesalamine and my inflammation is always worse in the rectum even though I have pancolitis at diagnosis. I have uceris foam, but that cannot be used long-term.

I am about to go on entyvio. I'm really worried about my ability to manage rectal inflammation without being able to tolerate rawasa enemas? What are my Long term options? This is so frustrating. I am feeling very hopeless.

Hydrocortisone enemas worked wonders for me, a little too good - but then it's a darn steroid and you will need to wean off of it. Mesalamine enemas don't' work for me as well anymore. They used to do wonders for me. Hydrocortisone enemas are also easier to retain I found. The intestine absorbs it and nothing comes out in the morning, no liquid
I also started entyvio in Novembe. idk.. I did not get the wow factor if any help at all from it. enemas are what got my situation under control. And I know how you feel, rectal inflammation is the worst. I start getting hemmies and they get worse and it snowballs. do you take any probiotics?

I understand your feelings Jane. I got very depressed when I was at the point you are in. I finally went on a low dose antidepressant which really helped. There are studies that link depression and the gut— in fact my GI noticed how hopeless I was feeling and gave me a very scientific explanation of what’s happening in our colon and how those elements are linked to depression (sorry but it was so technical that I don’t recall the terms he used) which made me feel better and not so reluctant to try an antidepressant. It can be very short term to get you over this hump.

I am hoping entyvio works for you and you will start to feel better soon. You should be able to use the foam for the 3-4 months entyvio takes. And remember I had many small improvements along the way in my journey to remission. So while 4 months seems like a long time you could see improvements before that.

Hydrocortisone enemas helped me as well, when mesalamine enemas did not. If it's too hard to retain the enemas, you might try hydrocortisone suppositories as a first step. That might reduce inflammation just enough to allow you use the enemas. I've found the suppositories are no problem.

Once in remission on a biologic you will not need a rectal med long term. You can use the steroid enema/foam if a mild flare creeps up.
