HI,

Sorry to sort of 'repeat' , as I saw a thread on this in June. But I am several months into Xeljanz, and very likely have associated psoriatic arthritis (pain in back, sausage fingers etc. Detected by my Derm but not yet confirmed by Rheum.)

I woke up 3 weeks ago with excruciating pain in left glute, no fall, no strain, nothing to justify it. Finally have doc appt on Monday (booking out sooooo far, she worked me in). It is absolutely crippling, spasming, etc. I saw mention of this enthesitis in the June posting.

Does anyone have any more info? I have done heat/cold/every sciatic and piriformis stretch on the internet and NOTHING.

PS, Xeljanz is still rocking!!! I hope this is not some side effect from the extreme nature of the drug, because after this, I am out of options to a colectomy. Thanks and everyone stay well - happy holidays.

Patricia, I did some snooping on the net, what I found is people with psoriatic arthritis can also have enthesitis. I read this on several of the sites dedicated to psoriatic arthritis. The description they give does not quite mesh with your glut pain, but who knows. You may want to check out National Psoriasis Foundation. It's not uncommon for people with an auto-immune disease to get another, our odds go up somewhat. At my worst with psoriasis, I had 4 different types going on it was terrible.

Enthesitis is a manifestation of all of the Spa's of which psoriatic arthritis is one. However so is IBD arthritis...in fact what a lot of IBDers call joint pain is actually localized enthesitis usually at the heel or at the shoulder. It rarely produces swelling.

I was admitted to hospital and put on 100mg hydrocortisone with poly arthritis which they said was secondary to my IBD. My joints swelled and I couldn’t walk on my feet. It started in my elbow, spread to my knee right foot, then left foot before I couldn’t walk. I’m not 100% sure if it was from my ibd and will probably never know because the hospital system is so incompetent where I live. I’ve been misdiagnosed 9 times out of 10 visits. I can’t find anything that relates this to ibd. My Gastroenterologist said he wasn’t going to focus on the cause at the moment. He said when my flare is under control he will refer me to rheumatologist. The pain and swelling went down with steroids.

That's pretty interesting because I got a piriformis issue seemingly out of the blue after going on Xeljanz too. I never thought there could be any connection. Mine eventually went away. I did lots of stretching and foam rolling and rested from exercise because I also got a stress fracture. If anything, I thought my issue was more likely related to steroids, but I have no idea. I went to a sports medicine doctor and he prescribed physical therapy for me but then I broke my foot and had to rest and my pain went away eventually. Mine doesn't sound as bad as yours though.

Hi TP177, I think in my case it wasn't the Xeljanz causing the joint pain and enthesitis (plantar fasciitis, neck, wrist and finger joint pain mainly), but probably a case of the Xeljanz not effectively controlling it, as I have a history of this sort of joint pain (mainly neck, wrists, finger joints, and a recurring shoulder problem).

I had the same problems before I went onto Humira, so rather than Xeljanz being the cause, it's probably more a case of the biologics Humira and Stelara covering more bases for me.

Is this the first time you've had joint pain / enthesitis?
