63 year old male. Dx’d one year ago. “Mod to Severe UC…descending colon, transverse colon”. Put on Humira due to history of pancreatitis. Going for a one year scope Wednesday. He drew blood. CRP is 58. WBC 10.8. Absolute neutrophils 8,800. No symptoms.

(Six weeks ago, all of my bloodwork was 100% perfect. CRP 1. White blood count 5.7. Neutrophils 3,700. No problem. No symptoms.)


What happened? Humera quit that fast? Maybe I have Crohn’s instead? (I’ve read where UC is not possible without “the butt part” being involved. The “Butt part” was not involved q year ago). A couple of “indeterminate” biopsies a year ago.

Or…

Do I have some sort of infection UC related or not? (Appears to me the blood work shows infection)

I’ve also read where CRP >50 can almost guarantee a bowel resection. Is this true? (Not to mention heart attack idea

p.s….how could a man go from no symptoms since diagnoses straight to a bowel resection in one year?

The bloodwork indicates active IBD flare. A flare can cause a significant change in blood markers in a matter of days let alone 6 weeks. However the fact that you are without symptoms is still positive; perhaps this flare is in a preclinical phase.

Absolutely not true that CRP > 50 guarantees resection. There are posts on here from people with CRP fr in excess of that who have completed recovered....i think my crp was once close to 150 in hospital..

Those blood tests could indicate an infection or could be a flare. My CRP is nearly always elevated because I have arthritis. Never heard that >50 indicates surgery. Can't remember what my highest was at my worst, but it was definitely higher than 50. The scope tomorrow will tell the story about how much (if any) inflammation you have. If you are flaring, there are lots of other drugs you could try before surgery would be indicated.

Let us know what your scope says.

IT, I seem to remember you posting about having strep throat not long ago? I wonder if the infection has completely cleared your system.

I've never heard of someone having surgery based on an elevated CRP, careful where & what you read on the internet, lol. Dr. Google would have had me dead many years ago. I've got other medical problems that can elevate my CRP, lots of arthritis.

Good luck with your scope, hopefully it will come back clear.

@straydog: I just want to thank you straydog. You always remember. There’s feelings of anxiety and vulnerabilities going through this a thousand miles from my home and family and support system. And we’re thinking alike on the strep. I began taking a Z-pack on the same day the blood was drawn. It’s possible that the white blood count has since gone down as my strep symptoms are completely gone. (The blood was drawn and I began the Z-pack on the 9th).

Wow CCinPA, I somehow missed your contribution and I’m sorry because it’s very useful information. I’ll be sure to try and let you know how things look after the scope. Very kind..

BTW…my GI called tonight. I told him LOL I was on the phone to my little sister in Knoxville. I told him I was telling her to pray for me because I think I might have sepsis. The GI said, “You’ve gotta relax man. Lol. You don’t have sepsis.”

Other than that, well…just another day at the Funny Farm.

IT, so you did your labs the day you started your antibiotic. I'm not a dr, but I am willing to bet that strep throat infection is what skewed your labs, elevated CRP can be caused by inflammation or an infection. If the stress of this is getting to you, go see see your gp & see if you can do another test.

I’m not sure I even have Ulcerative Colitis.

Last year’s biopsies came back as UC (with a couple of “indeterminates”). Heck, he even showed me pictures of it. That was when I was first diagnosed.

Ain’t there now. (And evidently, no signs of ever having had it).

Hey quincy,

I was drugged when he spoke to me after. (I was also very out of my element…IE…in the middle of Miami…a snowbird…lol)…so that was on my mind as well. What I do vividly remember is him saying, “Your colon is normal.”

My next move is to revisit an orthopedist down here in the morning who, several weeks ago, said a bone on an X-ray looked abnormal. He ordered an MRI for it several weeks ago. I did the MRI but never followed up as assertively as I should have been. I think I have SIRVA. (Possibly infected). That shoulder has hurt everyday since last August when I had a covid shot. It’s hurt in that same spot everyday since. (They’ve never even heard of SIRVA). Tomorrow, I need some direction.

If nothing else, as was discussed earlier in the thread, I need another blood draw.

I wish I had an internist. Supposed to see one Monday. (I’m tired of chasing rabbits and I’m no doctor).

As far as the UC goes, I’m putting this in the lap of the GI who first diagnosed the UC last year in Knoxville. Let him do the doctoring and solicit this colonoscopy report from this S FL GI. Monday week, I will be back in K-town. I have a network of doctors there.


Edit: I did ask him about the bloodwork. Before the colonoscopy he said “inflammation”. Yesterday, after the colonoscopy he said, “These things fluctuate.” I’m not buyin’ it. CRP 58…Something caused or is causing it. New order for bloodwork tomorrow absolutely. Lol. smile

It's possible there is inflammation elsewhere in the GI tract....its also possible this inflammation is from a non autoimmune source.....your bloods above could also be indicative of rheumatic autoimmune issues....do you have any ongoing joint issues

i agree with you that the chances of servere colitis returning normal archtecture so soon is low

do please remember that disease and the body does not follow rules....the bloodwork and scan results that we document as being indicative of health conditions are northing more that a collection of results that hold for the majority.....any true doctor worth her salt will look beyond the blood work and scans ...those who diagnose only on blood work and scans and give you no answers to ongoing symptoms...these doctors should be fired on the spot by you

Wow, crazy. That's great though about your colon. I'd be baffled as well. Glad you have a network of doctors in Knoxville. Definitely repeat the bloodwork. I'd bet it was elevated due to your strep infection.

Latest labs 3 days ago. Every single thing completely normal. Had to have been strep throat.

Also a clean colon.

This is all to the good. My other worries in life were feeling ignored.

Straydog and Sara tied for first place. Everyone else asked pertinent questions while supporting me by just answering and being there. Every bit as important if not more so.

You either research people’s past posts or you are a really caring person. (Maybe both?). There’s ALOT of caring on this site. You all really do seem to care.

The MRI showed nothing in the bone.

However…the gov’t has a program for “reimbursement” if one gets SIRVA. So, being the cynic (or pragmatist?) that I am…I have to believe a lot doctors do not want that headache in their lives. It doesn’t matter, the shoulder is much better since he shot it up with cortisone. That’s all I care about. I might have a radiologist that I trust look at the disc when I get back to Knoxville just to double check his work (that there’s no bone damage).

My wrist has arthritis IN THE TOP ON MY HAND!! (I didn’t know that could happen lol). There’s also some permanent swelling on the left side of that wrist. I think that’s probably something to do with my tendons?).
Whatever it is, the doc said “We’re gonna ride this juice as long as we can. Your wrist is a mess). Lol. (Bedside manner’s a bit different here than I’m used to).

It’s my left wrist and I’m right handed. So, that’s a blessing. 😊😊

Island time, I seem to have a knack for retaining many things I read, this was extremely useful when I worked. However, if I am questioned about something I did 3-4 days ago, I would have to stop & think about it, lol. Sometimes, I will look back at someone's post if it will help me give a better reply.

What is SIRVA, I tried looking it up & found nothing. I can relate to a hand being swollen on top. I have the same thing going on in my dominant hand, you can't see my knuckles & it feels warm to touch. Dr looked at it & said it's arthritis, no kidding, lol. Aging is not too much fun at times.

You contribute more than you realize to the forum, everyone does in their own way.

Sounds like arthritis can take many forms. (I bet that hurts)

At my age, getting through a day without a new physical problem or pain is the definition of a good day.
