ion:
For the colon biopsies, there is mild to moderate architectural
distortion present with no active inflammation identified. There are
no granulomas identified. Scattered mucosal lymphoid nodules with
germinal center formation are present.
For the anal canal lesion biopsy, p16 is negative, consistent with
low grade dysplasia/AIN I
Final Diagnosis
Colon, ascending and cecum, biopsies:
Colonic and small bowel mucosa with no pathologic change.
Negative for active colitis.
Negative for dysplasia.
Colon, transverse, biopsies:
Colonic mucosa with no pathologic change.
Negative for active colitis.
Negative for dysplasia.
Colon, descending and sigmoid, biopsies:
Colonic mucosa with mild chronic inflammation and minimal mild
architectural distortion.
Features compatible with quiescent inflammatory bowel disease.
Negative for active colitis.
Negative for dysplasia.
Rectum, biopsies:
Colorectal mucosa with no pathologic change.
Negative for active colitis.
Negative for dysplasia.
Anal canal lesion, excision:
Low-grade squamous intraepithelial lesion (LSIL/AIN I).
So does this mean remission? I'm waiting to hear back from my GI. I'm wondering if I always "only" had left-sided colitis. During my first scope, the surgeon doing it (lived in a small town with no GIs) didn't pass the splenic flexure because I was so inflamed that he didn't want to risk perforation. And none of the 9 scopes thereafter ever showed anything past that point that I can recall. My symptoms get so bad, I always assumed I must have pancolitis.
Odd that my rectum has no permanent changes. Maybe all those years of enemas? Thanks, quincy...

You're within a remission, no active inflammation found. Congrats!
Chronic architectual changes to your cells is noted on the left-side only. So, your historic extent of your UC is limited to your left -side only. That's sigmoid colon and above it. No symptoms from architecture changes, it's like a fingerprint or identifier that you are an IBD patient. Normal horizontal across your belly button and normal right side of the large intestine.
I'd ask your doctor about
2 points:
1.) Scattered mucosal lymphoid nodules with germinal center formation are present. It can be signs of a past infection. It's likely nothing but it's there in your report so I'd follow up on it.
2.) Ask about
the anal biopsy pointing toward a low grade dysplasia (precancerous changes). It's likely something they'll just monitor in future endoscopy, and nothing to worry about
now. I'd just bring that up as a point of discussion and verify it's nothing to be worried about
.
3.) Yes, there were no architecture changes noted in your rectum. Likely that's deeply healed and hasn't seen inflammation in a long time. Cells are replenished over long timeframes, erasing some architecture changes.

Thanks, iPoop! The anal biopsy was from an HPV lesion. I had that removed at the same time as the scope, but it was not directly related to my UC. It's something I've dealt with since 2015 on and off (found during a scope, no symptoms ever). It's always stayed as low grade dysplasia so that's good. I'll ask about
your other point though--thanks!

Thanks. Now I have to hope the surgery doesn't cause a flare though. It did in 2015, maybe a week or so later, and the surgeon said that's a possibility. I wish I hadn't even had that surgery done. But hopefully I won't flare! I will exhale in 2 weeks...maybe...lol.

If the architectural changes stated consistent with mild chronic inflammation, that would make sense. Are you still on oral mesalamine?
I don't know about
lymphoid nodules except they seem to be associated with infection of some kind (from what I read)....
Yes...you needed that lesion removed since it was HPV associated and presented with dysplasia.
All in all...looks like a great report. Can you use the enemas at this point?

Yeah, that's the part that confuses me as well, quincy. She still hasn't sent my GI the biopsy report. I copied and pasted it to him yesterday and they saw my message right away, but I'm sure he wants to read the official complete report from the doctor before interpreting it and writing me back. My GI always responds super quickly to me, usually even on evenings and weekends.

Yes about
the lesion but she told me all HPV lesions show as dysplasia and that the cancer risk was miniscule and there wasn't even good data on it and that all the other surgeons in her office don't even do repeated surgeries for that, so it really confused me over the years. I first had it removed in 2015. My body will just not fight off the HPV virus like a normal person, I guess. But yeah, I guess it's good I had it removed. I don't know.

I'm not in full remission according to my GI. Thank God I have him and didn't just listen to the surgeon (have I mentioned I hate her? Lol).
This is what he said after reading the biopsy report:
"This is pretty good. The biopsies of the left colon (descending and sigmoid), which includes the splenic flexure showed very mild inflammation. The degree of inflammation seems to be very minimum because it doesn't mention acute inflammatory changes."
Asked him about
the lymphoid node thing in a followup message.
Anyway, I'll take this result and be pleased with it. Lol.

My opinion...definitely into and above the sigmoid. How far would depend on how much stool is in the colon at the time. What dosage of delzicol are you on?
I also suggest fibre supplement and probiotics...
Im glad the GI clarified...because remission isn't what I see. You are doing well, however. Did you have an infection not that long ago?

I had a result like that once. I don’t recall now how far into remission tho. I remember my GI so absolutely no inflammation but the path report did show some inflammation but no active colitis. It might have been my second scope after my “remission” scope.

Thanks, q. I'm still on 4800 mg Delzicol but I miss some doses so should probably not do that anymore. I wasn't quite as vigilant with it as I used to be since feeling well on Xeljanz.
Not sure about
the infection. I thought it might be a possibility a month or 2 ago because I had diarrhea and some other pretty mild symptoms but all the stool samples came back negative for infections. I had a full work up. My calpro was pretty high. I upped my xeljanz dose for 3 weeks. My symptoms went away. I had been back down to lower dose for 10 days at time of scope. I don't really understand his comment about
the lymph nodules. I guess I'll be forever confused.
I saw some old post from you when looking up propofol. I hate propofol and they keep pushing it on me in recent years, even when I tell them what it does to me after it wears off (massive anxiety including intrusive suicidal thoughts, sobbing all night, no appetite....this time I felt drunk almost all day and evening). I just got ALL my old records and did well on versed and fentanyl in the past. Do you still request only fentanyl?
Flowers: Thanks. Good to know it can happen. Next time I will just have an actual GI do the scope. Im trying to find someone local again, at least for scopes, but healthcare seems pretty hit or miss where I live currently. This was an odd situation and I had a colorectal surgeon do it. Her written report says mild erythema but she told me she saw nothing which is weird because my GI said that note usually means inflammation. I feel fairly informed but the path reports still confuse me at times. And hearing different explanations from different doctors doesn't help! Then I try to Google things and sometimes confuse myself more. But thank god the internet and this forum exists so we can do our own research.

Sara...i get both Versed and Fentanyl. The Fentanyl alone wasn't enough actually. I've never had propofol during scopes.

Thanks. I'm not letting one more anesthesiologist force me to get propofol ever again. It also makes the cost $400-500 higher for the patient because the doctor himself can push versed...no need for an anesthesiologist.

I'm assuming that the area of surgery would have been frozen with only Versed and Fentalyl? That would be much more comfortable...😵💫

They told me they injected some local anesthetic at the surgery site. I don't know what it was. It didn't last long because I could feel it on the drive home already. But not bad pain, pretty mild. Last 2 scopes prior they pushed propofol on me hard too. Next time I will be more assertive on not wanting that.
