Had colonoscopy today, 7 months after starting Rinvoq…

Impression: - The examined portion of the ileum was normal. - Ulcerative colitis, in remission. Inflammation was not found on today's exam (based on the endoscopic appearance of the mucosa). This was graded as Mayo Score 0 (normal or inactive disease), improved compared to previous examinations. Pathology results pending. 🙌

How long have people been in remission for with Rinvoq? As stated on previous posts, i was in remission 3 years on Infliximab until my IBD team messed that up by taking me off it. Just hope this Rinvoq has the same affect when I eventually start it

I am on xeljanz which is from the same family of drugs as rinvoq and have been in remission for 4 years. I previously tried infliximab (including at a double dose of 10mg/kg), entyvio and azathioprine and none of them put me in remission although they all helped to reduce my symptoms to varying degrees.

Awesome. Well Azathioprine kept me in remission for 4 years before i come off it and infliximab to, so hopefully i respond well to Rinvoq. Sounds like a wonder drug for UC sufferers

Been in remission for nearly 2 years so far on Xeljanz. Boyfriend also takes Xeljanz and has been in remission for over 3 years.

Thanks all! The histology came back - Negative for acute inflammation and dysplasia (Nancy Grade 0).

So I am thankful for the trifecta (clinical, endoscopic and histologic remission).

But as we all know, don’t take this for granted as the disease can rear its ugly head at anytime.

The good thing about the jak-inhibitors (i.e. xeljanz, rinvoq, jyseleca) is that their effectiveness does not seem to be influenced by how well you responded to other drugs.

Do the jak inhibitors block one type of cytokine only or a while ratchet of them?

I am not a microbiologist but as I understand it, the JAK inhibitors target different components of the immune response than say a biologic does. The effectiveness depends on the role that component plays in your individual disease. So some patients respond well to JAK inhibitors but not to (e.g.) tnf-inhibitors, and some patients respond well to tnf inhibitors but not JAK inhibitors, and some patients respond well to both. I don't think that the JAK inhibitors necessarily block more cytokenes than other treatments but they do seem to be more effective in targeting the particular immune pathways that are involved in ulcerative colitis (broadly speaking).
