(I posted this on the Crohn’s forum but wanted to get advice here as well. Any suggestions are appreciated, I’m not sure what to do)

I’ve had Crohn’s for over 15 years and have been on Humira for about 10. I’m currently in a flare and my dr wants to switch my medicine and is suggesting Skyrizi.
I am hesitant to change as I’ve been well for a while (no flares for over 2 yrs) - I had to space out my Humira a few months ago so missed regular doses then was on antibiotics etc etc and am wondering if that caused/contributed to this flare. I’m on 40mg prednisone now and my colonoscopy shows a lot of inflammation.
Question: is it possible to get back on track with Humira or do I have to change meds? Has anyone experienced this?

Have you tested for antibodies to humira? That would give some insight into why you are flaring. How often do you inject? Sometimes you can get a response back if you go from 2-weekly to weekly injections. If you're in a severe flare I would tend to agree with your doctor that switching meds may be the best course of action.

I agree with poopydoop. You should get blood tests for antibodies and for levels of adalimumab (Humira) before switching. If you have high levels of antibodies, switch. If you have low levels of Humira, try more frequent injections (weekly). If you don't have antibodies and your levels are good, then it's just not getting the job done any more, and a switch to Skyrizi makes sense.

Thank you for the replies! I’m still waiting on the Humira bloodwork back. If my antibodies show something is off I’ll switch but am kind of stuck in a wishful thinking pattern right now where I’ll heal on my own (I’m also on prednisone now and that is prob what is helping the most even though it’s the most awful) My dr suggested switching to Skyrizi, Stelara, Entyvio, or Rinvoq. I’m scared of all these medicines/side effects on some level so just have to have a serious think about it and bite the bullet. This forum is a good place for sure when you’re trying to figure this out so thank you

I'm not sure any of those drugs has a particularly worse safety profile than humira. Only rinvoq comes with a warning about blood clots. 10 years is not a bad result on humira, although of course we want the longest time possible. I think my sister managed 2 years before she developed antibodies. She is on simponi now (another tnf inhibitor).

I don't have any advice, but wanted to also say that I don't believe any of those meds have a worse safety profile than Humira. Entyvio's safety profile is better. I don't know much about Skyrizi other than their slightly annoying commercials, and now I have their theme song playing in my head.

LOL, slightly is the understatement of the year. Those commercials are incredibly annoying, not slightly annoying! smile

My doc actually said I could switch to Skyrizi from Stelara if I wanted, and I told her no, in large part due to hating their commercials. Take THAT, Abbvie!

Skyrizi (or Rizankisumab) is probably the best and safest. Dr. Marla Dubinsky (Mt. Sinai, New York), one of the leaders in IBD in the world, has said that "she would shout from the rooftops until she is tired," to take Skyrizi. If I fail Rinvoq, I would go quickly to Skyrizi.

I do hear amazing things about skyrizi.
And if Marla Dubinsky and the Sinai folks think it is good I would try it for sure.

My doctor also loves Skyrizi and said that would be my next try if Rinvoq does not work (but it has worked very well so far- just very slow at symptomatic relief compared to the other meds I have used). He was more partial to Rinvoq based on all my arthritis and joint pain.
I think it is understandable to want to try humira for longer and short intervals. In my case, I took my Simponi shot Feb 15 but by the end Feb, started flaring. I called my nurse and she said we could switch to Rinvoq right away but it does take about 10-14 days paperwork wise as well as bloodwork and getting vaccinated but I said I wanted to take my shot March 15 to see if it was a fluke. I immediately regretted it since I kept worsening so they ordered antibody blood work. I had zero antibodies to Simponi but it just was not working anymore. I ended up started Rinvoq April 3 but totally wish I had skipped the March shot and started it sooner. I just really wanted to make sure Simponi was done since I started it Dec 2013 and that is a long time to be on a biologic.
