Pockets in my wrap

I have had one esophegeal surgery 30 years ago and 2 nissan fundo plications over the course of 17 years.  I was having reflux about 18 months ago and went and had an EDG and show that the wrap was intack.  I was prescribed Nexium and sent home.  THe symptoms never got better and in fact got worse.  I went back to my doctor 8 weeks ago.  He was going to send me for another EDG - but I live in Las Vegas and the day I was to go, they closed the clinics and the doctor's office forcing me into hell.  I was referred to a general surgeon who did a barium swallow and 2 days ago the EDG to have the surgeon tell my husband that I had pockets in my wrap and they contained undigested food.  I saw the pictures and he was right, I recognised dinner from 3 days prior.  He told my husband he didn't recommend surgery but he would do it if I wanted.  Question - how does one get poclets in the wrap or a plication?  Isn't this bad?

I have heard of getting food stuck in the wrap. It was one of the things the surgeon warned me about when we were discussing the wrap procedure. I hope to never experience it. My GI was kind enough to share the photos of my wrap area he took when he did an endoscopic procedure. The photo is looking up at the wrap from inside the stomach - it's actually pretty neat. Our stomachs are normally smooth on the inside with no folds or crevices. The wrap creates folds and crevices (pockets) that could allow food to get trapped in them. Imagine you have a piece of cloth - when it's hanging straight down, it's smooth. If you wrap the top portion of the cloth around a pole, you now have smooth on the bottom, but "crinkled" on the top. It's the best analogy I could come up with before my morning tea. I don't know what you saw when you looked at the photos and saw the food stuck, but I can understand how it could happen. Our stomachs are twisted into an unnatural position so nothing that happens from this point forward will surprise me.
