Hi Brian,
I don't know if anyone can answer that question--not even a doctor. I do know that some people improve their Barretts with medication alone. That's the whole point of it...to get control of the acid. I have definitely heard of Barrett's improvement without the need for surgery. They follow with a scope, and keep an eye on your esophagus periodically.
The regurg of food seems to be a quality of life issue as well. I personally didn't get very much help from maximum dosages of PPI's. I still had breakthrough heartburn, and my lungs were a mess.
The surgery isn't perfect. I've never said it was. However in my case, it was the only way to stop the acid/bile from refluxing into my lungs and creating a very unhealthy situation. At some point I may have to have a redo (remember, I had fragile tissue internally as a result of the high doses of steroids I'd been taking for my lungs.), but I'm ready for that. I can't go back to the way I was before the surgery. Any inconveniences (sometimes I can't burp when I'm bloated, some excess gas, not being able to swallow big pills. etc.) are well worth it in my estimation. I've suffered pre-op, and it didn't work for me. I'm just glad there was something they could do for me. I'll take it!
I know you've struggled with the idea of redo vs. take down. I guess you need to remind yourself of what brought you to surgery in the beginning, then discuss the options with your doctor, and try to figure out what's best for you.
