I found the answer on a nissen fundoplication website. I t is from the CO2 they put in you to expand the stomach to work. It is also from positioning.

I believe it is very common after any laparoscopic survey because of the gases they use to inflate the abdomen. I thought it was nitrogen they use though. I guess once you are vertical after the survey any left over gases travel up to the shoulder area and cause pain. Happened to my wife after a laparoscopic surgery.

Yep very common with lap surgeries of the abdomin. I had it with all 4 belly surgeries. Walking really helps get it to go away.
Take care,

My doctor warned me before my surgery that I would have shoulder pain afterwards. He said it was because during the surgery they turn you on your side and most of your body weight is on the shoulder.

The shoulder pain is from the CO2, but if you have a nissen with a hernia repair the shoulder pain also comes from gas in your system. Watch what you eat and WALK!
It definitely helps with the gas.
Best of luck.
