Upcoming surgery

I had a nissen fundoplication done back in 2008 and it was a major success for curing my heart burn, however fast forward to 2012 and i believe I have injured it again. For a few months I had raging heartburn which has gone down some but I'm still getting terrible chest pains and heart palpitations (yes have already done a full cardio work up its not that). Lately I have been having trouble eating more and my food isn't digesting as well lost a good 15 pounds but have been making slow gains back to my normal weight. I have been suffering pretty greatly and nobody wanted to do a re-do, I however have found a good dr by the name of george gillian who suggested something I thought sounded quite unique.

He said since my hiatal hernia damage isn't that bad when viewed from an endoscope he suggested using a procedure known as TIF to help reinforce my old nissen and move things back into place. No incisions sounds good to me and something is obviously wrong in my chest cavity everytime i lift something it feels like gears are griding in my chest its just too paintful and abnormal. However, the weight loss and lack of appetite are a pressing a concern now too so I'm a little confused if all these symptoms are related or not. Has anybody ever heard of a surgeon using TIF to recorrect an Old nissen procedure that is partially but not fully messed up. PLease give me your thoughts and advice I need all the help I can get to make this huge decision!

I am in York, PA, but I made the trip to see Dr. Gillian. Had all the tests, etc., saw him twice, but I backed out. I still haven't done anything because I fear my PVC's will get worse after the op. Funny thing is though only one doc has seen a small sliding hernia, yet my PVC's go nuts especially when inhaling deeply. That tells me that perhaps my stomach is in my chest cavity causing this, as I know the heart will skip when touched (from a cardiac cath a few years ago to rule bad heart problems out.)

Cmac, he recommended the same thing for me, though he did say I really don't even need that. I had just a tiny bit of reflux (a normal amount, apparently) during the barium swallow exam. I am feeling good now.
