Hi in May 2013 I had a large para-oesophageal hernia repair and the Nissen's fundoplication has become undone.  The surgeon said he is reluctant to operate again and i am feeling very scared as i don't know what to do now as the specialist is the only one that does this in my area.I am wondering if i go to a dietitcian to change my eating habits will that control this? My main problem is pain in the upper part of my body i do not get reflux and i have been told i have dysphagia now and my stomach is retaining food.  Has anyone gone thru simular problems any advice would be useful to me as my quality of life seems to be going lower.  I also burp a lot which i find embarssing especially when out!  I can not understand how it became undone as i lived on soup for six months and only started eating bread three months ago and i usually only have one slice per day even now. Any advice would be helpful to me. Thank you for reading.

I can't understand how the Nissen came undone. They do migrate sometimes. Did the hernia open up again?

A dietitian isn't going to know anything more than you do.

Hopefully Bill is reading. He's had several fixes.

Is there any possibility of seeing another surgeon somewhere else? I can only say I would get a second opinion.

At the least maybe you could see a gastroenterologist (not a surgeon) about getting the stomach to wake up and move food along. There are some meds that we can't get here in the USA that help with gastric motility.

Hi Alice and thank you for replying I live in the South Island of NZ and there is not any other surgeon who can do this as far as i am aware but i am going to try and find out more from somewhere.  The GP doesn't know of anyone else either so that is a worry.  I do not know why this has happened I am seeing the surgeon on 1st May he is overseas so i have to wait till then to get some answers. I hope anyone else that may have gone thru what i am going thru does see the post and give me any Information.

How did they determine the wrap had come undone? A manometry or barium swallow?

I have had mine redone twice with very good results, but I did have a different surgeon do the redoes than do my original. It does take more skill to do it right.

As Alcie said, it is unusual for it to just come undone on it's own. My 1st redo was because mine was done wrong and the 2nd redo was because of an esophagus stent they had to use when my esophagus tore and wouldn't stop leaking after surgery. It did tear the wrap apart. They couldn't tell it was apart with an EGD, but a manometry and barium swallow showed it was undone.

Is the other surgeon a thoracic or bariatric specialist?

I had a Barium swollow which showed the stiches had undone. I do not have the choice of going with someone else so it will be the same surgeon (if he agrees to re-do it) So that frightens me.
