I had a small sliding hiatal hernia, found many years ago but not a problem. I found out what foods "triggered" my reflux/heaving, eliminated them and lived with non-acid reflux (on PPIs) happily unless I ate too late in the evening.
Then 5 years ago I was in a high speed accident that shoved my stomach into my chest - paraesophageal hiatal hernia. A big part of my stomach sitting up alongside the heart, right through the diaphragm. I could feel it trying to roll when I turned over in bed. My reflux went from very tolerable to feeling like I was having a heart attack, and I'd had one of those a few years previously.
So, I went for the surgery after 4 months of testing and agony. Toupet - partial Nissen, because I have very "wimpy" swallow to start with. Long recovery because it's major surgery and I had other co-morbidities. Definitely worth doing! I still have trigger foods, but that's a sort of allergy (sensitivity).
I was never given any sort of "score." Surgeon said a paraesophageal hernia "won't go away on it's own," so he put me on the schedule.
