Nissen into Toupet Fundoplication Redo

Hi,

I posted at my 9 week mark about my 360 floppy nissen Fundoplication here https://www.healingwell.com/community/default.aspx?f=45&m=3378942 looking for some answers but no one ever got back to me.

So I am hoping I can maybe get some ideas here... If you read that you can see the issues i've had after my surgery. I am now at the 12 week mark and my surgeon is booking me in for a redo nothing has failed etc my body just cant tolerate the 360 so he's looking to make it into a 270

I am wondering has anyone had their nissen made into a toupet? and how did they find the recovery? Was eating etc easier (after the liquid diet etc) the cuts are fine and at 12 weeks my body in that sense is completely fine its the eating i still cant do and I go on holiday at the end of August and would love to be able to eat relatively normally again.

I would love to know as he is also saying I can choose to have it completly reversed if id rathe rdue to my daily pain.

This was with a 2nd opinion, my surgeon is highly regarded and only specialises in this type of surgery my GP has also had the same opinion. Ive now lost 2 stone and am still unable to eat and this is worrying for them as they feel my symptoms are the same as that of someone who has underwent a gastric bypass. I was stretched as the muscles weren't working and letting food/liquids pass the valve causing me to regurgitate what was in my throat.

My manometry tests were normal but he did the 360 as i was born without a valve at all.

At 21 right now its taking a toll on my whole life i've not had one day where i've been normal and i have an on sent of issues i never faced before. This is why my surgeon wants to make it into a toupet as he feels that after 21 years of never halving a valve my body is rejecting the 360 and isn't adjusting at all.

I did not have a redo but my original surgery was a Toupet not a Nissen. I agree that receiving opinions from more than 1 surgeon is important. Before I settled on surgery, I spoke to 3 different surgeons all with great reputations (and almost talked to a fourth). The first two recommended a Nissen. The third a Toupet and he is the one I used. (Not relevant to you but the reason for the Toupet is that I had a large pareasphageal hernia and, in his opinion, the Toupet provided for a better repair of the hernia). I guess the point is that two or more very good surgeons can have different opinions.

The surgeon I ultimately used does a lot of redo surgeries and he did say a number of them were redone because the original wrap was done too tightly.

I lost 15 pounds in about 6 weeks post-surgery and I am 58. At the age of 21, I would have lost a lot more weight. I had a lot of trouble swallowing and it has only resolved within the last week (about 7 to 8 weeks post-surgery). I did wonder if I had issues with the wrap but am now confident that things are working out. I would imagine the recovery from a Toupet is quicker than a Nissen (at least with regards to swallowing). I never did vomit and only experienced mild nausea.

Ultimately, I am not a doctor and cannot really provide much advise. Nevertheless, receiving multiple opinions before having another surgery, if you have that option, is worth considering.
