I also have GERD, gastritis, hiatal hernia, etc.
Over 5 years ago, I started to have a pain in the exact spot you are talking about, right at the end of the sternum, probably an inch or 2 to the right (so right under the end of the right side of my rib cage). It's like a dull ache (not a stabbing pain) that can be anywhere from a 1/10 to a 5/10 in terms of discomfort. If I put a slight pressure on it, it helps somewhat and sometimes I need lay on my right side in order to fall sleep. However, I never feel it go 'straight through to my back'.
I went to a few doctors about this, and got a HIDA scan, small bowel series, X-ray, ultrasound, etc. etc., and no doctor has ever been able to tell me what the pain is caused by or has helped me make it subside.
From what I've gathered online, *it seems* like it could be a manifestation of pain from the hiatal hernia. Even if the exact location of the pain is not where the hiatal hernia is, many seem to attribute it to that and mention that the location of pain is not always in the exact same spot with your organs because of the way things are connected.
