I am 9 months post Nissen to "cure" a mild case of LPR and I have been totally miserable since I decided to do the surgery. I have seen 7 different doctors and they all seem skeptical that the surgery failed. I will share my facts so maybe someone can help me:
Pre-Nissen:
No heartburn, mild sore throat that wouldn't go away.
Massive doses of PPI never provided any relieve. Diet, sleeping on a wedge pillow, etc didn't help either.
EGD showed gastritis and esophagitis (no Barrett's)
Surgeon proposed a Nissen fundoplication.
pH study showed normal levels of reflux (42 events, mostly (31) non-acidic events). DeMeester score was 7 (normal). Manometry showed complete aperistalsis of the esophagus.
Post-Nissen:
Day 4 after surgery I woke up with fluids coming up to my mouth (bitter taste). Since then, my mouth is on fire 24/7.
Did 2 barium swallows - both showed mild esophageal dysmotility and delayed esophageal clearance
Repeated pH study - 10 reflux events - 0 acidic and 10 non-acidic. DeMeester score was 0.8
Repeated EGD showed mild esophagitis, consistent with reflux.
Repeated manometry showed 80% of successful peristalsis
Last week I started having constant heartburn and chest pain. Went back to massive amounts of PPI, Gaviscon, Tums - nothing seems to help
When I drink water, I can feel the fluids slowly going down, immediately followed by a growl - similar to a clogged sink. Swallowing is difficult. It seems to me that the Nissen is too tight.
I am going to see a new surgeon on Friday. Would anybody recommend to redo the wrap to a partial fundoplication? I would really appreciate if anybody could share your story if it is similar to mine.
