The initial manometry test I did was fine. I had it done at the Mayo Clinic in MN. I would imagine that my swallowing is still fine and it's the reflux that causes the issue. For example, when I drink water, I can hear a gurgling, croaking-like sound sometime for a minute or so after I swallow. The discomfort isn't in the swallowing itself, but rather the time after I've swallowed.
When I previous surgery failed, I had similar issues. I had a 3cm hiatal hernia, which isn't particularly dramatic, but it still presented itself rather profoundly in symptoms. There was damage to the crural diaphragm after the first failure, and my wrap was mostly undone and partially migrated, and both had to be repaired. Whether or not these problems resulted in explicit dysphagia or simply mimicked it may be negligible; from what I read in research articles, I get the impression that the symptoms reported in instances of wrap failure present themselves differently from those in hiatal hernias in patients who haven't had previous operations.
That said, I think I'm at a point where may not be worthwhile to speculate much further. The simple facts of the matter are that I've been having issues with reflux, burping, chest pressure and pain. The digestive tract has its own (enteric) nervous system, and it doesn't necessarily "communicate" in a way that is easy to diagnose independently. Once I'm in a financially stable position again, I'll just have to run back through the gauntlet of tests and find a professional with experience in nissen revisions and failures.
