Two to three hours is not enough time for your stomach to fully empty before going to bed. I am intolerant of PPIs, and have adopted a somewhat radical routine of not eating past 3pm, and going to bed 12-1am. That way I can be certain that my stomach is empty, and that the Gaviscon tablets won't be diluted by food and have maximum effect. If you're eating small meals, at least give yourself 4-5 hours.
How much is your bed elevated? Six inches or so is about the maximum you can safely raise on blocks, but that's not really high enough. 10-14" is better. Get a foam wedge from Bed, Bath, & Beyond, and build it up a bit with old towels or blankets. It's important that the wedge goes from your hips to your head -- don't just raise your head with pillows, as this will make the reflux worse. I understand that you're probably having reflux episodes during the day, too, but everything you do to reduce your daily exposure will help. If you have one available to you, it actually might be a good alternative for the time being to use a recliner.
Prepare your history ahead of time before you visit your new doctor. I know exactly how it is when you visit a new doctor -- you're nervous, and a lot is happening, and a lot of facts can be forgotten. If you present a written document to him, it can become part of your file, that he can look to later if necessary, or forward on if you are referred elsewhere. Include all symptoms, dates, medications, doctor and ER visits, etc. If you're a good typer, it will only take a few minutes in Word, and print it out.
Best wishes. Don't be discouraged if you don't get a good feel for this doctor. Many of us, including myself, have been through several GIs before we find one who is familiar with GERD symptoms and treatment. Keep a level head, and try to be as informed as possible.
