Have chest and arm pain

Hello, have recently had some weird health issues and could use some advice. I quit smoking for about 4 years and recently went back do to stress. I however, noticed I was having sever chest and arm pain, not putting smoking as the cause. I when to the ER as I was concerned about a hart attack. They found no issues and sent me home. After a few more days I linked the pain to smoking and quit. The pain subsided within a few days but now returns for short periods every week or two. This has now been going on for months and now my neck glands seem to be swollen for no reason?

Any Ideas would help.
