I'm thinking the fiber/fructose/etc may be a bigger issue, as for how long she's been on the PPI's, surely there's been some changes to her digestive tract, so may have SIBO, and therefore may have to do the fast tract diet. The whole website kind of screams 'hey I'm a "doctor", do this and it can cure everything, buy my book' that you see plastered all over the internet. A 'too good to be true' vibe that I'm very wary of. There's another book Dropping Acid, that looks promising, but seems a bit opposite from FTD.
I'm the same way with oil, I don't detect it much cooked in to things, but if I pan or deep fry, or have an oily salad dressing, I can feel it in my throat/esophagus. Oil in general I'm wary about, I've read too many negative things about it over the last few years. The most conflicting thing is the Mediterranean Diet, where a lot of olive oil is consumed. Is it actually, OK, or does it just not matter since they are less sedentary than Americans, and/or eat better in general? Who knows.
Did you ever have any surgeries? I've read that there are some procedures that can be done to tighten the LES: Nissen Fundoplication, Stretta, and LINX.
I'll look into amitriptyline, though may not be able to take that since she's pregnant.
She does have depression, and with the pregnancy, work, and not being able to go out and be social due to the very loud chronic cough is all causing more stress/anxiety. Vicious cycle. Trying to work on relaxation and meditation techniques.
Have you cut out wheat/gluten? She's been tested negative for celiac, but I'm wondering if it could still be a problem, since it's one of the few foods that she does have somewhat consistently-crackers with nut butter for breakfast, noodles, bread, etc.
