Hi Guys

I am looking for a 3rd opinion on my GERD scenario as the first 2 I got re Surgery were both at opposite ends of the spectrum in terms of approach. So I am confused tbh.

Can anyone recommenced surgeons on the NHS they have direct and positive experience with, anyone specialising in the UK would be great. I have mild motility impairment yet a good oesophageal reserve so I am getting a difference in opinion on approaches to fundo Surgery. I have a 4cm HH also.

I will travel anywhere for someone whom specialises in GERD surgery.

So I have been offered a 360 wrap lap or open my choice (following all work up) and a 180 watson wrap.. I do not have a super strong swallow, but the High Res manometry has been properly reviewed in both cases... I kind of want to go for the 360, I am nervous of side effects with dysphagia and but I think the 'safer' choice is 180.. but I am confused and they are both polar opposite..

I am also from the North West UK. I used stockport nhs trust who were really good with me and my fundoplication. they offer a full wrap or the partial wrap. I had the full one and havent really suffered ajorly from side effects but im currently experiencing a recurrence of symptoms due to gastritis but thats just part and parcel of my condition. I was discharged the same day as the operation and other than a few fare ups of gastritis have remained symptom free! (I had the op in 2013)

Have you made your choice about which surgery to have?

I notice from this Forum that you have considered LINX, Endostim and various variants of the fundoplication surgery.

If you have not yet had surgery, where was the quote for £13,000? I know of a surgeon who does LINX; Endostim; Stretta and Fundoplication. It might be worth having a consult with him to review your options, even if you opted for any NHS available procedure. My experience is that he would be happy to do that to help you think about your options. He is not just after "private patient surgery fees". There would, of course be a private consultation fee, but they are quite modest by London standards.

Since you have a hernia you are not eligible for some of the alternative repairs. My surgeons quit doing them anyway, as they don't have the great success rate of the varieties of Nissen.

With the swallowing difficulty you should talk with the surgeon about a partial wrap. Let him decide if you can tolerate the 360, or maybe something in between though. I have an extremely "wimpy" swallow, so I had the "Toupet" wrap (270 degrees). That was 2009 and still works great!

A wrap will not correct wimpy swallow or other swallowing issues or food sensitivities. That's fine.
