Can PPI's stop working? Or just a rebound!?

Hi,

I am a 31 year old male who has suffered from reflux since 2013. I initially got prescribed PPI's back then having had an episode of unusual heartburn. I got prescribed PPI's however when I finished my course of PPI's I still had heartburn.... this progressed and I ended up being on PPI's consistently since then to control my symptoms.

Over the last 4 years or so I have been managing my symptoms with Esomeprazole 20mg daily. However during this time usually once or twice a year I seem to go through a difficult period where I have weeks of painful heartburn/reflux and it seems like the medication just stops working!? Then something seems to happen eventually and it Esomperazole starts to become effective again. I am currently going though one of these phases now where I am incredibly sore in my chest and have a consistent pain all day. It has been nearly 2 months now and I wondered if anyone else had this and what I could do to get the medication working again?

I tried swapping to Omeprazole 20mg twice a day to see if that worked but it didn't...

There are some people that it doesn't work for. 20mg is a low dose though. I would take 40mg every morning for a few weeks to see if that helps. Also, not sure if you know this but it needs to be taken on an empty stomach 30 minutes before a meal.

I took 40mg Nexium for awhile then switched to 60mg Dexilant. I had surgery 3 weeks ago though cause it wasn't helping and I had a hernia as well.

I think PPI's works different in each case. And yes 20mg is low dose. 40 mg is normal dose for GERDians. My reflux actually does not shut down by I think any PPI. Some say because if less sleep and stress it persists. When I wake up in morning I have cough and burning sensation in throat, sometimes ulcers too develop in mouth which goes away in 2-3 days without treatment. Sometimes when throats hoarseness/pain is more i can't talk much (than by stretching) as it strains the vocal cord I think. Sometimes also I have jaws dull pain below teeths. Many times I feel burning in mouth and throat while eating because of reflux irritation.

This all happens to me twice or thrice a day. Lol.

I started now rabeprazole 40 mg twice a day which I changed recently as earlier PPI had side effects of Gynecomastia, so GI doctor changed the prescription.

I am currently on 40mg Esomeprazole daily (doctor has increased this from 20mg whilst I have this bad spell of reflux/heartburn) and 150mg x 2 Ranitidine daily. I am still having really bad heartburn/pain in my chest though at the moment confused

I spoke to a consultant last week who feels that a large portion of my reflux is due to anxiety and hypersensitivity of the gullet due to how anxious I feel about this. Does anybody else have experience about this?

Does anybody else have rebounds whilst taking PPI's? How long do they normally last?

I wouldn't feel as bad knowing that even though I still feel pain and reflux that the PPI's & ranitidine are doing something and are still protecting me? I have read that PPI's cannot work for some and not others and that either way my proton pumps will be producing less acid whether I feel like they are or not.

So does this mean what I am feeling would not be causing as much damage and maybe non-erosive reflux? I'm just freaking out here and just want thus relapse to calm down!

I live in Melbourne (Australia)I was diagnosed in May 2016 and prescribed pariet for 6 months. Later in Nov 2016 had one more biopsy and the diagnose was same. This time I got worried and took the matter seriously. I refused to go with PPI as it doesn't cure you. It just reduces the acid level. I started to avoid food and drinks causing acidity. I stopped drinking all liquids (alcohol, packed juices, soft drinks etc) except water and milk. Started eating food without garlic, tomato paste and tomatoes itself, all types of fried, fatty and junk food, at evening eat only cooked vegetables or a luke warm glass of milk, reduced the quantity of intake. And followed Ayurvedic food recommendations for Vatta-Pitta. In addition every morning 30ml of Aloe Vera juice with water 1 hour before the breakfast.
Plus added every morning 30min Pranayama Yoga.
And all it worked.
In Feb2017 also did Panchkarma cleaning in Kerala.
Now on 27/07/17 I had the endoscopy and biopsy done and it was found that I no longer have Barret's.
I think both the diet and Yoga worked nicely.

Has anyone tried slippery elm. I broke two capsules open yesterday put them in water and drank them and soon as I did my stomach starting hurting. I don't think this is suppose to be a side affect or does anyone know if it is. I really afraid to take anyone more after that happened. Maybe taking one would be better till my stomach gets used to it. I know taking that way it got quicker in my system. Any suggestions on what to do.
