Make sure you tell the endocrinologist's office that you were hospitalized recently and have been told you are in a ketoacidotic state. Call the ER and ask if the doctor who saw you can make an urgent referral, and see about getting paired with a social worker who can hlp you. Definitely enlist the help of a friend or relative to come with you to any appointments you have and to make sure you give all the pertinent details and get the help you need.
When you go to the endocrinologist, make sure to bring a few things with you: a) a list of ALL the medications you are taking and their dosages and the time of day you take them; and b) all of your recent medical records, or at least the phone numbers/names of the physicians you have recently seen for ANY condition, along with any lab results (sounds like you might have a notebook full).
The problems you're facing sound like they stem from a lack of communication and a lack of organization among your providers. I will always stress the importance of having a family practice doctor above all. When you have different health issues, it can be hard to keep them all straight, and it's really not your job as a patient to see how the issues fit together - that's what your doctor went through all that education for - but this is where your cooperation is needed, to ensure they are all on board. This is especially true when you start throwing ER visits and hospitalizations into the mix - things get crazy because you'll likely see a different doctor every time!
