Your symptoms do sound like many of the symptoms of anxiety but I do understand your fears of the "foggy vision". Sounds like you have followed up with several doctors but wondering if you have seen a Ophthalmologist or an Optometrist. Both are very good at what they do but the Ophthalmologist is a physician -- doctor of medicine (MD).
Optometrists and ophthalmologists sometimes work in the same office and co-manage patients.
Re the smoking - from what i understand, people who smoke cigarettes are at increased risk for developing cataracts. It causes a number of vision problems, including blurry distance vision, sensitivity to glare, loss of contrast and difficulty seeing colors. However if you have had an eye exam a cataract should have been discovered.
