I think it might be useful I have and am doing it and it has helped me immenesly

I take ativan and I have been on 3 a day as well when one of my other diseases gets going I am on 3 ....2 mgs of the ativan to keep blood constanly calm or try .........and stress levels way down ........
I am only taking my ativan and /or diazapam when absoulteky needed I have come along way I must say with the cbt and support oof ppl like Karen and the rest of the gang here
Sometimes I did find the ativan at the onre 3 times daily did nothing for wahtever rason other times it worked
I have been on them for more yrs then I care to count lol
I would try the dosage adjustment with doc knowing and or try another SSRI
PPL are having success with Lexapro and Effexor I have seen here so you might want to mention this to the doc to see if there is another you can try
Also the sublingual ataivan ( under the tongue) work faster and get into bloodsttream immediately ( like a nitro for Heart attacks ya know )
You may need adgusting there as well
Self help is another biggie and I am glad you do read the self help books
Coping skills are a MUST IMO I too am like Karen I dont think just MEDS cut it.....and SUPPORT system is a must IMO
