Anytime you change the dose of Prozac, you need to give it a good 3 to 4 weeks to build a steady state blood level. Prozac has an extremely long half life, so unlike the rest of the SSRIs, it takes a few weeks to build a theraputic blood level anytime you change the dose. But increasing the dose can cause immediate side effects like nervousness, nausea, etc. If you're not having bad side effects, I would stick twith the 40mg dose for a good 4 weeks before determining if it's working or not. Too many people quit antidepressants too soon due to this lag time. If you stick it out for a month, you may be pleasantly surprised to find yourself feeling much better. I was taking Celexa 20mg, then 30mg, and wasn't feeling much of anything, and when I hit the 40mg dose I thought I would try something else. But after about 3 weeks at the 40mg dose I noticed I was feeling better. My advice is to stick it out, and if you feel anything really unpleasant, call your doctor asap.
