I am really sorry to hear of your current situation and the stress that it is causing you...after all, anxiety is what you are trying to find relief from...not increase ! :(
I really think that confronting your P-doc about what you have read concerning your Rx's is crucial for a couple of reasons.
For one, you need clarification. Often times interactions are listed for med combos...but doctors prescribe them together despite this due to individual circumstances. You may not be at a particular risk, due to your gender, age, general health, ect...
However, for another person's individual situation, it might be a poor choice...thus the warnings must be printed.
If indeed this was an oversight on your P-docs part, then you need to have the validation of this for your own peace of mind. This will let you know that you need to seek another doctor for your care, and will bring you some sense of closure about this.
I commend you for trying your best in your recovery efforts. I can tell it was not easy for you to stay positive and take the steps that you did.
I hope that this does not sour you for finding what will eventually work for you. You can find another good psychiatrist if need be.
I encourage you to also explore non-medicinal options such as therapy and self-help techniques to help you along in your goals
