Hi IP,
Being aware of our breathing, or focusing in on shortness of breath is a common anxiety trait. We often, without being aware of it, rapid/shallow breath...causing unproductive breathing and tight chest muscles.
Falling out of shape happens, and can contribute futher to these feelings that you describe while ascending stairs, or other exertion.
Doing your best to cut down/quit on the cigarettes, getting some low impact regular exericise, and practice breathing exercises will help.
All are an effort I realize...but one that I am sure will be worth taking for your health and peace of mind! :)
