As far as facing one's fear goes...this is going to vary according to the individual.
For me, it meant returning to the sites of places where I had experienced panic attacks. It also meant continuing to go to work everyday...though I was often symptomatic, and thought I might have a health related episode during the exertion from my classes. (I am a dance teacher)
For you, it may mean something entirely different...but exposure therapy can be tailored for individual needs and be really effective.
Keep doing the things you already putting into practice...
...but seek to learn some new skills either through self help, or with a therapist.
CBT can be really helpful with the challenges you mention ...(and we have thought challenging worksheets in the Resources that you can start with right away!)
