You've got two things going on: pain and anxiety. That's a very tough combination! I don't have significant pain, so I can't be of any help there, except to empathize.
about anxiety, I had moderately debilitating anxiety. I've tried Pamelor (one of the tricyclics like you are taking) and it didn't work; in fact, I couldn't sleep well when on that drug. I've also taken Zoloft, which was okay, but I didn't stay on it too long.
Ten years ago, when my elderly parents needed care and moved to be near us--and job stress--I knew I needed to take something to dampen the anxiety. Citalopram, which is FDA approved for anxiety, worked well after the initial adjustment period, and has ever since, although I upped the dose from 20 mg to 40 mg about 5 years ago.
I have not seen a psychiatrist, though my internal medicine doctor and sleep doctor both suggested one of the newer anti-anxiety meds (citalopram). I have seen a psychologist to learn CBT (cognitive behavior therapy). The medication to me is a anti-anxiety foundation that I have built on with other coping skills. It's been hard work, but worth it. On a non-anxious/calm scale of 1 to 10 most days are 7 or above. The excrement still hits the ventilation system now and then, but that's life. The important thing is that I know what to do to get back on track.
